Individual Learning Plan Forms
Individual Learning Plan (ILP) - This form is designed for the new teacher and mentor to collaboratively develop an Individual Learning Plan for the new teacher. The form is to be used along with the teacher reflection forms to help determine specific areas for professional growth for the new teacher. Once the form is completed, a copy is to be sent to the Teaching and Learning Office. Timelines for completion for the ILP can be found in the D200 Teacher Mentoring Program Calendar.

Individual Learning Plan Reflection Form - This form is to be filled out after the new teacher has completed their ILP and reviewed the progress with their mentor. The form is to be reviewed with the new teacher's building administrator. A copy of the form is to be returned to the Teaching and Learning Office by the end of the year.

COLLABORATIVE ASSESSMENT FORMS
Collaborative Assessment Tool - This form is to be used after peer observations are completed. The form is designed to help the conference reflect on the teaching practices observed to determine areas of strength and areas for improvement.

Observation Form Year 1 - This form is used to document the required peer coaching observations. Once the observation and the collaborative assessment is completed, this form is to be signed and returned to the building administrator.

D200 ESSENTIAL TEACHING PRACTICES
This document contains the essential qualities and minimual attributes of a teacher in Woodstock CUSD 200. These practices are expected to be observed by the end of the first year of teaching by teachers with three (3) or fewer years of experience. Teachers with more than three (3) years of experience are expected to have these observable qualities at the end of their first administrative observation.
