Physical exams are required for all students entering Pre-Kindergarten, Kindergarten, grades 6, 9, and for any student entering District 200 from outside of Illinois. Proof of immunizations against communicable diseases is required by the Illinois Department of Public Health for all students entering Pre-Kindergarten, Kindergarten, grades 6, 9, or 12, and for any student entering District 200 from outside of Illinois.

Illinois Health Requirements for Grades PreK, K, 6 & 9

The state health form linked below should be taken to your child’s physical and/or immunization appointment with their physician. Physical exams and up-to-date immunizations are due on the first day of school.
Illinois Dental Examination Requirements for Grades K, 2, 6 & 9
Dental exams are required for students entering kindergarten, second, sixth and ninth grades. For the 2023-24 school year, these dental exams should be completed by May 15, 2024. Please use the form linked below when you visit the dentist. If you do not have access to dental care or need assistance, school dental exams are available at:
